You are not going to believe what the cause of this problem was. Just by accident I found the cause. I was working on some settings in the web.config for my editor 'CuteEditor'. I had some issues with relative paths and 'remove server name' settings. Then I noticed that there also a setting 'UseHttpHandlerCacheImages="false"'. After spending countless hours reading over some suggestions here in this forum and stuff I found using Google I began to understand how Forms Authentication works...at least a bit

If I set UseHttpHandlerCacheImages="false" appearantly images that are called for by a client are no longer handled by the HttPHandler and therefore is authentication failing on those forms. Correct me if I am wrong....

I will inform Adam (CuteSoft) about this because I don't anybody else searching for weeks to fix this. I took a look at the server logs to be 99% sure about this and they are still clean after running for approx 1.5 hours with the changed web.config.
