One problem is that you cannot rely on the same user always having the same IP address.  When organizations use certain kinds of firewall/gateway it is quite possible for a succession of requests; from the same user; at the same browser; to be come from different IP addresses.  In the past, not sure if this is still true, AOL was notorious for this.

A ClientCo I worked for earlier this year had such a firewall so I know they still exist.

I think that, without some kind of physical token - like SecureID - it is not possible.
