so the "0" value I found as default in some third party modules is a non-sense.. I thought it meant cache indefinitely (until memory fills up and oldest items or lower itempriorites are removed).

About the page output caching, I did some research on that subject for my custom CMS and there is a very neat solution to serve cached pages to anonymous and not to registered (or some roles):

This can be handled in the ResolveRequestCache adding it to every BeginRequest through a callback with an http module

From what I read here:  http://inspectorit.com/cms/dotnetnuke-cms/dnn-page-output-caching-works/

it seems that a similar behaviour is built-in with DNN. About Page Output Caching:

  1. Like module caching this feature will not operate when a user is authenticated.
  2. Only works for GET requests and not POST requests
  3. Search engines do not get cached pages.
  4. Pages are cached by TabId and up to  250 variations will be cached based on querystring differences.
  5. You must explicitly enable page output caching on every page by setting it’s duration.
  6. In page settings you can set a querystring modifier to either enable or disable page output caching. But this setting only works on a page by page basis with no way of having a host or portal level setting.

ok, I haven't used page output caching myself - the blog post states clearly that this feature is DNN Professional (evoq) only.
