Are you able, via SQL Trace, verify that the upgrade is actually hitting the database and there are no errors?

if you look in the Event Viewer are there any errors logged during the upgrade process/attempt?

Lastly, are you in a Hosted environment and has your Host done any recent updates to the server or Hosting environment?

Your issue would be expected if somehow SQL Server access is being corrupted

Your question put me on the path to the solution which ended up being that the upgrade zip was incomplete. I did a compare of the full install to the upgrade and noticed a lot of missing files, I redownloaded and voila!

All is well, except the login css is still hinky!

I re-downloaded the zip and unzipped, then copied over. I use Beyond Compare to make sure the files look right, works like a charm.
