I had deleted those already, but I'll double check those as well.

Indeed as mentioned in the linked topic above, I did have several old DLL files (not the Survey module, but SQL releated) that appear to have been creating the compile error.

This site started life as a very early 3.x site and although I'm not sure why those files remained after all the upgrades we done, once they were removed, the Survey module and the Site load without error.
