i have uploaded it to the templates folder. And I know its the right folder because I can save exported content and it shows up there. How do I get it to pick up the template?

moduleActions -> Scan for new templates

After you uploaded new templates, form and List might not detect them directly, but require a new scan for new templates:

 Hi

It shows up everything except the Scan for new templates. I have worked the issue. When I first installed the module I created a basic form. Once I deleted the basic form the option for scan new templates came up and it works. Thanks for the help cheers

Into what folder do I place a template?  I developed a template on one site, and now wish to use it on another site.  I uploaded my template to /wwwroot/Portals/_default/Templates as well as /wwwroot/Portals/0/Templates.  However, when I run the Scan for New Templates, I get No templates available.  ??

Did you rename the template? The name of the file MUST end with .dnn_userdefinedtable.module.template
