Hi, 

I too got the same problem with Form and List module.  First I followed Stefan's tip to solve the issue, but it didn't work.

Later, I went to "Admin ---> File manager ---> Templates", and downloaded the saved template and removed the content node as said by Stefan. Then, deleted the old template in the file manager and uploaded the edited one. Then, I cleared the DNN cache. You can do it by going into "Host --> Host settings". Then at the bottom, u can find a button "clear cache". Now try to apply the template. It works.
