In DNN 4 make sure the Gallery album has its permissions set correctly in FileManager.

I don't suspect permissions to be the culprit here since I could add images in some cases.

I'm having the exact same problem on DNN 4.4.0 - but when logged in as host and I go to the Admin - File Manager - There is only the Portal Root folder.  It's obviously not picking up the Gallery folder (which I can see is there on the server).  And if I try to add a "Gallery" folder, I get the error that says it already exists.

When viewing the folders in the file manager you have to first refresh the folders using the small blueish refresh button directly above root folder.

Other than that I'm having the same problem and I'm using the modified gallery from jegers.com on 4.4.1

After using the Gallery Module in various versions for a long time, I ran into a bug that I think is present in all versions and am pretty sure of the cause.  If your pictures DPI is less than 96, the Gallery Module will upload the file to the sources directory, will create the basic picture to show, but will not create the thumbnail and therefore the picture will not show in the album.  All this happens without showing any errors.  I ran into this because a friend gave me some pictures that he reduced to 72 dpi and I could not load them up until I changed the dpi to 96.
