you might try to rerun the SQL scripts (ordered by version number) inside the sqldataprovider directory for appropriate setup of the module.

I receive this error when running the 03.03.00.SqlDataProvider.

I am using UDT 3.3.6 and trying to rerun the sqldatproviders as suggested. When I run the sqldataprovider 3.3.0 I receive the above error.

in this case, a later script has already run and modified the data. just ignore this issue, we will try to avoid it in the next version.
