I fixed my problem. There was an error in the code. Not quite sure where. I downloaded a few downloaded some developer tools for Firefox and used them to pull up the errors and fixed them 1 by 1. There are still a few errors but my surveys are now working properly. Thanks for pointing me in the right direction with the Javascript errors.

You forgot to add the options because either the "New Option" words are just confusing (Just "Options" is fine) or the developers forgot to add "Yes" and "No" options as defaults.  To fix that:

1. In "New Option" text box, type Yes, then click "Add Option" link.  Repeat the same procedure to add No and other options you wish.

2. Click "Update" link.  It should work now! Good luck.
