It's pretty much a stock install of the skinobject that Telerik provides. The only real mod we made to the skinobject was a few lines to allow users to see items that require a login so they know what they are missing. The rest is just a bunch of CSS.

BTW Telerik support has been incredible to us over the past year thats why I've been singing their praises in the forums. It's all forum based but response times are normally under a day and right on target.

I have found another alternative solution which is using the Inventua SideMenu. It can do almost everything I want except there are still some limitations in configuring the css. Apparently it is cheaper comparing to telerik.

Yeah the Telerik stuff is pricey. Fortunatly they have per developer liscensing and we do enough volume to make it worth it. I think each of their standalones is around $250 and their suite of 18 controls runs about $1000.

I have also had good luck with the inventua sidemenu -- you can see an implementation here.  For that site -- the inventua menu was the only one that would fill the requirements (although I did not try out any telerik products...)
