We need some advice and some help with a problem we have encountered in our DNN portal running the latest 3.x verion ans Catalook Shop latest 5.x version

Our customer wants a smal popup window who displays a form where the customer can fill inn his name and andress and a small comment , then click send and the form data should be emailed to the shop ordering office  eg orders@shop.com. on each product.
The problem for us is that we want to capture the part no and part name in that form to mkae it as easy as possible to order a product sample. :)

We can add html code in the descrition field for the part and are willing to hard code the part no and name and cut &past on each part no just to satisfy our customer.
At the portal form mster is installed but we cant add modules inside the Catalook shop .

Is there anyone who has a hint what we could do here ?
