What version of DotNetNuke and what version of the Survey module are you using? You will want to use the latest version of the Survey module and it only works on DotNetNuke 4.03.05 or higher.

version of DotNetNuke : 4.4.1

version of Survey :  Survey_04.00.60_Install.

I can't add model Survey. I don't understand.

What version of the Survey module do you have? You can download the latest version here:

I download at http://www.dotnetnuke.com/DotNetNukeProjects/ModuleSurvey/Downloads/tabid/866/Default.aspx

but, I don't load model. An error "DotNetNuke.Services.Exceptions.ModuleLoadException: The stored procedure 'dbo.GetSurveys' doesn't exist."

database does't exist tables: GetSurvey, AddSurvey. Why?

Config DNN_Survey.dnn ? please help?
