I was recieving this same error, here is the reason why.  I am using MSSQL 2005 and had just updated to 4.5.  The survey model uses the Leagacy section, and even though I had replaced the connection string , I didnt realize that section is commented out.

So: 1. if you updated recently, did you change your connection string in both places?

  2. Did you move the close comment to a more appropriate position?
