There is a new bug that was just identified "Module uses legacy connection settings" I think you will want to check your web.config and make sure you updated the database connection string in both places and try the install again.

The conn string looks fine and works for everything else.

The error states "Failure SQL Execution resulted in following Exceptions: System.Data.SqlClient.SqlException: Column names in each table must be unique. Column name 'IsCorrect' in table 'dbo.DNN_SurveyOptions' is specified more than once." which means to me that it's connected ok, but the sql has tripped.

In any case, since it looks like the latest SQL executed ok, I'll try to use the module on a hidden tab.

I looked at the sql in the file and I see this:

ALTER TABLE {databaseOwner}{objectQualifier}SurveyOptions ADD
 IsCorrect bit NOT NULL CONSTRAINT DF_{objectQualifier}SurveyOptions_IsCorrect DEFAULT 0
GO

I could see it causing a problem if the table (and the field) was created in a previous install and wasn't removed. Can you uninstall, Verify that the SurveyOptions table has been deleted and try again?

That did the trick. I uninstalled version 04.00.70 of the Survey module, checked that all tables had gone from the DB, then installed. This time no errors.
