I think they are in the /app_code/Survey directory.

This particular project is used by DNN as a model module development - see Module Development document in the documentation download (available for 4.4.1)

There you will find most of the explanations, as well as answers to many of your questions - it will actually direct you to copy SurveyController.vb, for example, to app_code folder, etc.

Read that document and you'll be much more clear on this project.
