I have a fresh install of DNN 4.5.1 and set up a survey.  When I click to submit I always get "The survey is incomplete.  Please complete and resubmit."  I've deleted the questions and made just a single question with 3 possible answers with only one selection allowed.

What's up?  How do I get this to work properly.
