I had exactly the same issue. Upgrading did not help. What solved it was to select one of the answers before submitting the survey.

I'm getting this behavior as well, but only in Internet Explorer. Working with DNN 4.9 and we have the most recent version of the Survey module (4.6).  We are using "Vote tracking via cookie."  Even when all questions on the survey are responded to and "Submit Survey" is clicked, we are shown:

The survey is incomplete. Please complete and resubmit.

And the resutls are not saved.

Again, this behavior only occurs in Internet Explorer (we're testing in IE 7.0.5730.12). Firefox and Safari seem to have no issues.

If we revert back to using "1 Vote/Registered User" for the Vote Tracking, then the survey works in IE as well.

Mea culpa! I figured it out. There was a <form> tag elsewhere in the HTML of the page that wasn't properly formed. That was interferring with the Survey module and causing this behavior.
