I need to add a simple javascript call on the body onload event.

If I add it to the Default.aspx DNN throws and error.

What is the best way to add a body onload event to a site?

You can add your own onload handler by calling the ClientAPI.AddBodyOnloadEventHandler as discussed in this document on Page 10

I was hoping to do it without changing any VB code. I try to keep customization to a minimum so that upgrades are simpler.

Any other way to do it?

Not sure I understand the problem with doing it in the vb code.   I am not suggesting changing the core.  Simply adding it to any page (whether in a SCRIPT runat="server" tag or in a code behind), it doesn't matter.  If done in a module or a skin object, upgrades will be unaffected.
