That's the "More info" template for the gallery. Change it back to default. I had one stuck like that for months before I figured out what it was... I'm, not entirely sure what situation that template would be useful for either.

What is useful is that you can make your own templates just by copying a template folder and giving it a new name.

You could try and restore defaults:

1. Open your site
2. Open some text for editing in FCK
3. Click through to "Show custom editor options"
4. Scroll down and click Clear
5. Close the window and click Refresh Editor

The actual setting you want is on that options screen under "Editor Skins". Make sure the image browser is set to "Default".

Note that the setting is made either per-Instance, per-Module, or per-Portal and you will need to check what options have been set for each of those by clicking on the radio buttons at the top of the window. You have to scroll down and Apply the settings for each as well. The workflow is confused.
