Yes point taken - I already use hosttracker.com to ping and report uptime for low traffic sites but this internal request method seemed a little more elegant and lateral solution. 
Of course heavy pinging makes stats a little skewed to non technical viewers.
Also most things are worth a try (on a test site).

line added to web.config file in httpmodules as you suggested but same error.

Maybe how medium trust is configured is a factor too.

Hi Ian,

2 things:

  • I've uploaded a new version, which fixes the medium trust issue, so if you upgrade, you should be able to see the module's interface
  • That interface will probably only tell you that the handler is installed and provide you with a link button to get rid of it: 
    the module's interface's only purpose is to install/uninstall the handler in the web.config.

If you have the module installed and the web.config set up, then you don't need to keep any instance of the module at all. The keepalive should have started already. I suggest that you check your application start / application end event logs to make sure it's been running correctly.

Can you confirm it's ok?

Not if you are pinging keepalive.aspx page on the root of your DNN site, and if you are using an external stats package, some will allow you to exclude domains from the results.
