Is there a custom form module out there where you can solicit responses and have it email the form contents to someone? or store the data in a database.

Here is an example of what I need to capture:

Name

Address

Phone

Email

Comments

Prayer Requests

or for some other form of an event registration type item I might want to ask questions like number of children, yes no answers, multiple choice selections from a drop down?

Is there a module out there that will do such a thing?
