Good to see some progress!  I guess we'll have to see how 4.5.4 works to see if that was the main culprit behind the problems...

I was trying to track down some common denominators between different problems with 4.5.x installs that I've seen personally and described on the forums.  One of the recurring issues appears to be working at a module level (some modules work, while others don't), where things are not displaying correctly depending if someone is logged in or not, which is making me wonder...  While I haven't read through ALL of the forums, I have noticed a trend in the several forum areas that I do subscribe to, and was trying to gather information to piece it all together.  If I can determine a pattern, it should help with the diagnosis.

If I read that code correctly, that code segment is only run during the login process, or a re-login if the session has timed out?

When it's run I'm not 100% positive on but I'll check with Shaun and the rest of the core team at our weekly chat tomorrow. It is possible that it's related but if the if statement fails the user is then logged out and the cache is cleared of that user. Users would notice if they were suddenly getting logged out.

User is still logged out on page refresh or change.  Need only click on the home page to be logged out.  Initial authentication seems to function but I have stopped testing since user is logged out on a click of the home page.  See my test results:

I just loaded up the test site that was broken yesterday and the user isn't getting logged out no matter what page I go to. I haven't stepped through the code to see how often the account re-authenticates yet but I will later this evening. I'm on my way out the door but wanted to get an answer in on this.

sconard, can you debug this install with VS2005 at all?
