Is the "choose your skin" option like up above on the DNN site an easy thing to add to a DNN portal? 

Here you can choose large font, small font, full width, medium width.  How is that done?

If you are talking about the Icons to the left of the search control - they are skin objects from Snapsis.com.  They have to be installed under modules and the objects need to be rferenced in the skin.
