  • StarterKit is for developers using Visual Studio Express for Web Developers (VWD)
  • Install is for new installations and can be used for upgrades as well
  • Upgrade is for upgrades of existing installations, if you don't want to upgrade the modules as well
  • Source is for developers, who would like to tweak the core code or have a look at it

The differences between upgrade and install are, that install contains web.config file (if you use it for upgrades, make sure to back up your existing version) and all current versions of modules, upgrade package only contains links and Text/HTML module.

I suggest to upgrade to 4.3.7 first and to 4.5.4 afterwards (when it will be available in a few days).
