I installed DNN 455 for the first time and it worked like a charm, except for the fact that we were unable to set up Windows Authentication for database (all of our other apps use Win Auth). Which user id needs to be given appropriate permissions for Windows Authentication? I am a domain admin and dbo but Win Auth failed for my id too.

Other larger problem that we have is - we want to uninstall DNN 455 and do a clean install.

So far this is what I did and did not work:
Deleted DotNetNuke database
Deleted Virtual Folder from IIS
Deleted DotNetNuke folder from the file system.

At this point, I believe all the objects related to DNN 455 are gone from our environment.

So now i try to do a re-install and after deploying all objects as they should be, when I go back to http://localhost/dotnetnuke, I get forwarded to http://localhost/DNNIntranet/Install/UnderConstruction.htm and nothing else happens. What do I need to do to re-install each piece of the portal back on same server?!

For the windows authentication to the db, if you are not using impersonation it will be the ASPNET user account on the hosting machine.  If this is on Windows 2003 it will be the NETWORKSERVICE acocunt for the machine.

As for the re-install it sounds like you removed the items that you needed.  Are you 100% sure that you dropped and re-created the database that you are using?  Did you update the web.config to point to the NEW clean database file?

I'm having the same problem, except I'm trying to restore a 4.5.4 site that I had working recently. I have a localhost setup that was working fine until we wiped the computer and reloaded everything. I made sure I installed SQL Server first, then VS 2005, I've got ASPNET and NETWORK SERVICE permissions on the web directory (do I need to check to replace the permissions on child folders?), and have made sure the connection string is right and the database has the appropriate user. It's installed in wwwroot and IIS is set to ASP.NET 2.0. I'm stumped and my site has been down for days. Any help is greatly appreciated.
