I'm in exactly the same situation and have done exactly the same things. Glad to see I'm not alone but does anyone have any idea why the other database objects aren't being created? I tried making a new SQL login as a db_owner and still have the same problem. It's only creating the Version table it's procedures...

Hey I found the answer here:

I just changed <add key="UseInstallWizard" value="true" /> to false, hit ctrl-F5 and a whole new window immediately came up and installed everything for me. I'm sure it probably just chose a default install but the portal is now working... Hopefully now I can upload it as is to my web server and it'll still work.

I'm having the same problem. FIle permissions pass. Database connection tests pass. Get to the next screen and all hell breaks loose when it starts working with the javascript install script. I don't know if it matters, but all throughout this process my images aren't showing up. All I'm getting are the red x's ... which made me think it was a permissions issue, but ASPNET and NETWORK SERVICE are configured properly. ARG I tell you, ARG !!!

My answer came in the form of pointing my localhost website to the directory that housed the dotnetnuke install. After that I had to follow this step and I was off and running.

Exactly the same problem here, but I was installing dnn (480 and 482) from a local system and from a developmentenvironment (vs2005 or vs2008 both didn't work)
Installing them from an IIS system did the trick....
