You'll be wanting to use forms for more than this alone, as well, down the road.

There are two form applications that I use regularly. Easier but still feature-rich is Form Master. More complex and fully-featured is Enterprise Forms. Form Master is very easy to learn and use and packs a lot of punch. Enterprise Forms - very capable and powerful but quite a learning curve. At least for slow-minded folk such as yours truly. But it does amazing things, once you get the hang of it.

Take a look at FormMaster from Code 5 Systems, LLC.

Very easy to use, support is extremely good, and the module is priced very fairly.

Steve has done a great job on this module!

I suggest our Indoo SQL Grid (http://components.init.si). It has built-in (date, required, integer etc) and custom validators, date picker for date fields etc. This basic form funcionality can be made by clicking only.
