Where might I find Nuke Feedback? I did a search for it, but turned up bare.

I'd highly recommend xmod as it really is very powerful but has a very simple learnign curve for quick forms. 
The other option we use is Formmaster as so many have mentioned. again this is used for a large number of datacollection forms within our university.

There are several simple feedback forms around.. I've tried them all and there's always something not right or missing.

Nukefeedback is the best basic feedback form. It was created when the core feedback module went completely off the rails a while back. Get it from Orizonti.com

DoneIn does a cheap one that initally was a very worthy step up from Nukefeedback, but it has a problem affecting one's editor personalisation setting. I'm afraid I can't recommend it now.

A good form builder just for feedback/contact forms is Interactivewebs Feedback Designer , look for it on Snowcovered or the DNN marketplace. Your module editors can use this without further intervention from an admin or page editor.

From there on up you're looking at a broad selection of form designers, each having a relatively steep learning curve. They're all total overkill for a simple feedback form however. Browse the forms section at Snowcovered for an idea of what's out there. Everyone has a favorite.. I hate them all :)

Mitch's suggestion is a good idea for a free one.

Xmod has a nice cloak feature that uses javascript to hide email addresses from bot's.

Right at the moment I would also agree with Robax - I hate them all.  But it looks like this may (hopefully) change in the next six months as numerous new versions are suppose to be coming out.

I tried to take a look at Nuke Feedback, but is it just me, or is the navigation at Orizonti.com kind of jacked up?  Another back sign was when testing their online demo of the form, I got a critical error after hitting send...
