I'm trying to add a CheckBoxList to my DNN site.  Has anyone done this, or have suggestions?  Maybe I'm just being a little dense on how to accomplish this within DNN.  Also, as users "fill out" the check list, I would like to be able to persist their entries and have the ability to recall them later.

Any help or guidance is appreciated.

Is this part of an existing module? It sounds like what you are looking for is to save to the database and then report on that data. This may require some customization.

Thanks for the response John.  No, this is not part of an existing module (that I know of).  I was hoping someone could tell me which module might help me accomplish this.  Sounds like maybe none of the free modules would work.  Perhaps a custom forms package?  I want the users to have the ability to enter multiple 'instances' of a particular set of checkbox values, and allow them to be able to recall each 'instance' later via the web site.

For example (not my specific use, but it gives you the idea):

Check all of the things you did today:

1.  Ate breakfast
2.  Ate lunch
3.  Got up early
4.  Went to work

In this example, users could recall the 'instance' of the checkboxes for each day of the year.  In my app, it's not based upon day, but just their username itself, so they can recall all entries that I have stored for them.

Make sense?  Probably needs something more custom than DNN....

There are many schools of thought on this one.  I can show you a few things if you want outside of the forum here as I have done this for one of my clients recently and it worked very well.

No granted there is no DNN specific modifications that are needed so you could do this just like you would with any .NET app.
