Site is setup to allow AD Authentication. At least for some users. It gives the AD prompt each time. I have found nothing that works as far as the Single Sign-on recommendations. I have unchecked allow anonymous access and checked integrated Windows authentication. I did the same settings for WindowsSignin.aspx. I went to the file in file explorer and added Domain Users with Full Control to the file. Local\Administrators has full control, Everyone has full control, System has full control and Local\Users has Read & Execute and Read access.

Here are some settings from web.config.

Only some of AD Users are being pulled over. Some are being pulled over with Display name and some are being pulled over with no display name.

The provider inserts a record using minimal information when it cannot find the account using ADSI or LDAP.  (if I've mangled that someone chime in) If you do not get three "OK"s when you set-up AD authentication, you can actually see this in action.  In this case it sounds like intermittant access to AD which could come from how folks access your site or from some other issue.  There are other posts which look like DNN may have trouble creating an account when using impersonation and AD but in these cases you seem to get stuck in an endless loop.
