Am I required to have a SQL database to setup or install DNN?  If not, what should my web.config look like?

Create a database in SQL named DotNetNuke and assign a dnn user account as dbo.  Update the web.config in the 2 locations to use the property account, then run /dotnetnuke/install/install.aspx?mode=install.  This should run the installer for the whole site as well as the full db install.

Do I have to have a database? I cannot find anywhere on the site or the documentation that it is a main requirement. Nowhere does it mention that your host must be able to have SQL databases.

you either need to create an empty database and grant dbo permission to a server login or use the Express file based access, which is prepared by default in DotNetNuke installation. I usually prefer the first method, cause you can simply access the database with database studio as well.
