I have the same problem - cannot see the DotNetNuke project template after running vsi file.  I can see that DotNetNuke.zip was installed under ...\Visual Studio 2005\Templates\ProjectTemplates\Visual Web Developer\VisualBasic.  I checked - Visual Web Developer was installed and enabled.  There is Ajax Tool Kit project template in the same directory.  However, after clicking New Web Site I don't see DotNetNuke Web Application template under My Templates.

I can see however DotNetNule module item template under Visual Basic /Web projects.  I imported project settings several times (VB Developer, Web developer, etc.) - it didn't help.

What can be wrong?

Problem solved - one needs to pay attention to selecting language when selecting a template (and not to rush posting message:-)).  It is a shame, though that C# is not widely supported by DNN community.
