Okay in your case it'll only synchronize the first time a user logs in. To fix that you need the DNN AD Fix for DNN 4.5.1 which you can find under DNN Modules on http://dnn.gmss.org. Each version of the fix listed coresponds to the version of DNN.

I have applied the patch to my site. I have verified my Web.Config and I get no errors when I run the Windows Authentication. It finds all my domains. But for some reason I still cannot get a seemless login.

Any ideas why this would be?

I've heard of troubles with mutliple domains before but can't remember exactly what they were though I think it's the same (or similar) to what your seeing. It's something that I need to look at but don't have a multiple domain setup to test against.
