To address a few items you mentioned.

1.) I agree it doesn't make sense, the one thing they do around here is after a while of posting and proving that you are not a spammer or someone that needs moderation they will disabled moderation for your account.  This is a very helpful thing, and from what I tell they typically tend to do this sometime between 50-100 posts on the forum.

2.) You can change the default setting for threads per page, and posts per page.  If you click on the "wrench" icon at the top of the forum that is your "My Settings" page.  If you expand the user settings section there are settings there that you can modify!

3.) The decision makes are the leaders of DotNetNuke Corp as they are the ones that maintin the site.  That would be Scott Willhite, Joe Brinkman, and the others.  Typically when threads like this pop up one of them will come in and give a statement.

If you look just above the forums search box, you'll see a small downward-pointing triangle of the type used to indicate a hidden menu.  Hover over this and select the 'My Settings' option, you should then be able to find the place to change the number of threads and posts on a page.

Whenever this question comes up on any forum, the answer is always "because it works."  The choices for most forums are to be totally unmoderated,  unmoderated posting but cleaned up by moderators after the fact, moderated or priveat/invitation only.  Private forums only work for supporting specific products, where the purchaser is a forum member.  Totally unmoderated get trashed and fail within a few days to a few months.  The other two are equal, except that the first requires more moderators reviewing content and the latter delays posts until the moderators approve them.

High traffic forums with fe moderators usually have to moderate posts.  Lower traffic with a higher number of volunteer moderators can moderate after the post.  But then you have to manage the volunteers.  Mercifully, in most forums at least, moderation occurs reasonably quickly.  Less so if you're several or many timezones remote from the majority of moderators.  The solution most of the time isn't to unmoderate the forums, but rather to moderate more often.

Or, become better known as a responsible member of the community and you get granted greater access earlier.  Earn the respect you're asking be granted by default.

I, for one, am ok with amount and process of moderation here.  Posts here generally stay on topic and aren't cluttered with nonsense, blathering, or spam.

Seriously, what's the most time you've ever had to wait for one of your posts to be reviewed?  ...5 minutes? ...20 minutes? ...an hour?  ...a day?  It's seems like that would be an important factor.  I've never had to wait more than a few minutes before my posts are posted (knock on wood), hardly making this forum 'unusable'.
