I don't think anybody will be able to tell you exactly what notation to use because we cannot see the actual physical location of the image in question or the definition of your IIS setup.  I mention the later because if "PCMT_Portal_DEV" is a virtual directory under your localhost default site, then it should not be included in the path to the image, it is actually the root of the site.

For instance, I use the following notation to access images on a site I manage:  src="/Portals/1/Images/rotation1.gif".  In this case, "Portals" is right under the root of the site, even if I access the site with the following URL "localhost/DNNTest".  In this case, the DNNTest part would not be included in the path to the image, it is already there as part of the URL. 

To test this, I would enter in my browser "localhost/DNNTest/Portals/1/Images/rotation1.gif" and the image shows up.

It is probably the length of your file name.

I ran into the same problem (as far as I can judge from your description) and shortening the file name to less than 8 characters did the trick.

I must correct myself: it doesn't seem to have to do with the length of the filename. What eventually !seems! to have done it for me was copying and pasting the image file (and renaming the image file).
