Definition.  Clean install on the new site. Our site was built and delivered to us by a 3rd party company that has since gone out of business and left us on our own. The Module (Dynamic Forms) was pre-installed when we recieved our site. Now we want to rebuild it on a new host.

you need the original install package and install it on the target site.

If you do not know the original version of DNN and you have only the database.bak file with a folder that housed all additional content apart from DNN install content can I restore a site? And I assume I must restore to the same version, not the most current? I can restore DB locally and look into the DB with a SQLExpress install.

it will be tricky, to restore your DNN site from an incomplete backup. First restore your database and get proper DNN version from Versions table, highest number has been the version your site was running. Download this package from dotnetnuke.codeplex.com (select version in the box on the right hand side). Install this version into a different database on the same database server, using same database owner and objectqualifier. Compare ModuleDefinitions table for missing modules and install it on the new site. now switch database and move over portals folder. Good luck.

I actually have the orginal working site, but during upgrade (not manual - I let GoDaddy do the upgrade from 4.5 to 5.5) something happened because all I get is ...

Line 131:
Line 132:            ' all of the logic which was previously in Application_Start was moved to Init() in order to support IIS7 integrated pipeline mode ( which no longer provides access to HTTP context within Application_Start )
Line 133:            Initialize.Init(app)
Line 134:
Line 135:            'run schedule if in Request mode
