Can I use user defined table module

http://www.dotnetnuke.com/Products/Development/Forge/ModuleUserDefinedTable/tabid/877/Default.aspx

to create a form with javascript field validation?

For multiple languages, how should I design a form?

Does it belong to create a new module?

sorry, there is neither Script support nor multilanguage support currently included in the UDT module, you might start to create a custom module for your purpose.
