The page you linked to is a custom ASP.NET application, not a DNN app and thus that isn't a module.  It wouldn't be *too* difficult to build something like that custom, or you could probably accomplish a similar app through XMod or ListX (I don't have experience with either, but from what I see they are both pretty in depth).  You might even be able to put something together with the UDT (User Defined Table) module.

Fooberichu is correct Xmod or ListX both could accomplish this - as well as a couple other modules -

like Enterprise forms which is the one Michael G. will vote for!

Which one depends on your level of programming abilities, how much data you wish to work with, and how you want it structured.

Point taken, I know I've been pushing it a bit. I was kind of surprised at how often Enterprise forms wasn't mentioned when it was an obvious candidate, and thought it should be more in the general conciousness of the community. I know there are other modules that can accomplish some similar tasks and respect them as well.

If anything, however, I've not done EF justice ...

That was a joke Michael!

I think EF is a very good module.  What I find interesting is that most all of the forms modules for DNN have their own "niche" so to say.  While several of them can do many different things they each have their strong point that they are very good at.
