I can not find this file. Survey.ascx.vb.  I installed the source version of DNN 4.02 I can see the Survey.ascx but not the .vb version.

You can get it from the DotNetNuke 3.2.2 Soucre download.

It was inadvertantly left out of the 4.0 release, but there is no difference.
