There are basically 2 reasons this could happen:

1. Your ascx is not actually being shown. You can test this by just adding a bit of plain text to the ascx to see if that shows up.

2. Caching is non-0. In that case DNN does not run your code, but pulls in the latest rendering of the module from cache.
