no, you cannot use Text/HTML as a form, but there are free forms modules available, e.g. Helferlein form at www.helferlein.com.

Next Version of UserDefinedTable will have forms option as well.

Interesting....

I went a whole new route and just created an external ASPX page; linking to it from a Text/HTML module as a pop-up window.

At which point, I created an HTML email code (which was more than a pain in my neck) that sends me the information filled out within the form.  It was tough, but I found that worked as a work around, rather than hunt down a specific module, or even bothering with modifying one.

Unfortunately, I still am quite the noob with this stuff and will be seeking guidence, regularly.
