I have the same issue (admin/folders/file manager all look correct, but user profile has no option/link to upload).

Was this resolved?

There is no issue, its a matter of not understanding what to do. You can read the user documentation which outlines everything. One note, user avatars are uploaded from the user's "My Settings" and not their visible profile page.

Ok, it is work well now.

The option below must be unchecked if we want the user upload their own avatar.

At Enable Avatar Image Pool, check check.gif to provide user's with a folder of avatars from which they can choose, or uncheck uncheck.gif to disable. This option requires Enable User Avatar to be checked. This option removes the ability for user's to upload their own avatar, restricting them to selecting from those you provide.
