On a survey form, I ask 40 or so test questions that go along with a presentation on another page.  At the end of the test questions is a submit button, that will email the results to me.  What I would like to find out is how I can set up the page or submit botton to show me what user sent me the test.  As it sits right now I get an email from myself to myself.

The Survey module does not e-mail results so you are already using a customized version.

Michael, I'm not sure I'm describing the form that I am using correctly.  Can you tell me how to attach a picture or file to the post to show you?

I do not know how you can upload apicture, but the functionality you describe is not part of the Survey module.

How about a form Master.  I have 40 questions with drop drown menus to select the correct answer.  At the end of the question is a submit button, which when you click on it it sends an email from myself to myself.  Our IT Deprtment can't figure it out, so that's why I'm here.  As you can guess I don't do all the programming everyone else does in this forum.
