There are some minor issues with the skin, but none of them related to what your experiencing (more about the position of elements)

Check that you are not using anything that blocks Javascript, i.e. ZoneAlarm.

I am having the exact same probem with a fresh install of 4.09.01.

I had it running yesterday without this issue, went to recreate to verify the install procedure for some custom modules.

Everything went well except for this runtime error, same place ( Line 44 ), points to the same issue, the search icon in jscript.

installed on local with Minimal Skin.

Here's how i addressed the runtime error.

 

When i installed the install package i was getting Errors saying "a script had already run". to fix this i commented the following code in the web.config file:

<add name="ScriptModule" type="System.Web.Handlers.ScriptModule, System.Web.Extensions, Version=1.0.61025.0, Culture=neutral, PublicKeyToken=31bf3856ad364e35" />

After you complete a successful install go back to the web.config file and take out the commented tags, save and refresh.
