I've installed DNN 4.9.1 on an x64 Windows 2003 server with SQL Server 2005 Express.  I ensured that SQL Surface Area Configuration allows Remote connections using named pipes only.  I created a new database and db user with password and gave the db user db_owner.  When I run DNN 4.9.1 install, I used the following settings:

SQL Server 2000/2005 Database.  Unchecked Integrated Security.  Specified a database name, user ID and password.  Run as DB Owner is checked.

Using these settings, I receive the following error message:

Connection Error(s):
Index #: 0
Source: .Net SqlClient Data Provider
Class: 20
Number: 2
Message: An error has occurred while establishing a connection to the server. When connecting to SQL Server 2005, this failure may be caused by the fact that under the default settings SQL Server does not allow remote connections. (provider: Named Pipes Provider, error: 40 - Could not open a connection to SQL Server)

Can someone advise me on what settings to change?

what type of connection string are you using? If your server is an IP address or a URL you need to allow tcp/ip remote connections.

Also, if you're using a non-default instance name (e.g. a default instance would be "mymachine" a non-default would be "mymachine\dev" , then you need to have the sql browsing service running otherwise the remote connection cannot be routed.

Finally, right click your database server in sql server management studio and select properties, click the connections tab and ensure that "allow remote connections to this server" is checked.
