I don't expect the width and height be hardcoded in any module, but some may used fixsized surrounding div's.

Currently the image path is not stored in a variable, that can be configured, you might want to suggest this in the public issue tracker (2nd from top) at support.dotnetnuke.com.

Please see my post, here: http://www.dotnetnuke.com/Community/Forums/tabid/795/forumid/109/threadid/281578/scope/posts/Default.aspx where I explain how to define custom module action icons programmatically, in both DNN 4.9.1 and 5.0.  Please do read further though, where I have discovered a bug that is currently being addressed by the core skinning team.
