I have seen DNN do this from time to time. We put up a brand new install and the caching was messed up. We could add a page and it would disappear. If you navigated to Admin --> Site Settings, the page would show up in the menu and you could click on it. You could naviagate away and the page would disappear again.

I am not sure where the problem is but I can confirm that DNN 4.9.2 still does this. In another post somwhere, I asked about this and was told it was a permission issue but all permissions were good. I use DotNetPanel to install all of our installs now. I simply removed the portal and Database and re-installed and it worked the second time. It does not happen often but I have seen it happen randomly.
