Yay... Take Me to the Community!

I need to collect user feedback about communities in 50 locations, and I want to post that data in appropriate sections in all 50 sections. For example, I want users in Community A to tell me the best hair salon, best kids' schools, etc. in the location. Each piece of data would be placed appropriately in each section. So Community A would have a section on Best Salons, Best Schools, and each piece of data from one user survey would go to the right spot. I would like to administer this survey from one location on my main site, but I can create the form in each community if necessary so it posts data only in that community.

Do anyone have any recommendations on the best way to do this? Do you have any examples? Your help is always inspiring and appreciated.

I assume you will need to use a custom module or a generic module.

Does each community has its own portal or do they share the same portal and user base?
