Is there a possible way to use this form and List MuduleV5.0  in dnn 4.9.* ?

This is a great module with superb enhancement from old UDT.  Cheers

 No, I apologize, there is no DNN 4 version of the module. I'd suggest to wait for DNN 5.1.x be ready for production use and upgrade - there are a number of great enhancements in the new framework as well.

So there is no easy way to grab the form bit and use it with the old UDT?

There have been significant changes between UDT 3.5.1 and FnL 5.0.0 which prevents from backporting.

If you need a forms module, I'd suggest to use free Helferlein forms from www.helferlein.com.
